The Coaching Application form is required for all persons desiring to be a coach of any DYSL sponsored team.  Additionally, a background check and additional training is required by Babe Ruth League and the City of Dover.

DYSL requires this form for all players prior to each event for 2021 to help protect our players, their teammates, and their families.  It is developed from the State of NH guidance on Amateur and Youth Sports.

Responding “Yes” to any question disqualifies participation in the event.  Please keep your player at home, safe and healthy.
